About the Role

• Inspire and support consumers through formalised peer support, practical assistance, tailored education, information, referrals and support to learn the skills and knowledge to build resilience for mental and physical healthcare needs and to engage in their community.
• Provide guidance and mentoring to the Peer Workers within the localised work area.
• Utilising lived experience expertise and a trauma informed care perspective of accessing services to advocate with consumers, enhancing their awareness and knowledge to maximise system navigation and reduce power imbalances.
• Engaging consumers of Continuing Care Teams through targeted conversations to maximise their self-determination in their recovery journey.
