GENERAL DESCRIPTION: Coordinate and oversee the AWARE program on site and case management implementation. Develop strategies to assure retention and help students with program participation and personal growth. Increase and improve participant’s access to culturally relevant, developmentally appropriate, and trauma-informed school and community-based AWARE activities and services.
TASKS AND ESSENTIAL JOB RESPONSIBILITIES:
1. Implement program curriculums, coordinate trainings and ensure quality program implementation.
2. Develop strategies to assure increase mental health awareness/knowledge amongst people who interact with school-age youth so they can understand and detect signs and symptoms associated with mental health disorders, substance-use and co-occurring disorders.
3. Coordinate with community schools to ensure sufficient recruitment and coordination services for the AWARE program.
4. Assess needs and social emotional wellbeing of the youth; and design the strategies to support them in the access of services within the AWARE program and external referrals.
5. Provide positive support for behavioral health and connect targeted services for those students who need additional support, and intensive services for those in critical need.
6. Connect school-age youth, (and their families) who may be experiencing behavioral problems, including Serious Emotional Disturbances (SED) or Serious Mental Illness (SMI), with critical services.
7. Work with the AWARE Program Coordinator to assure full compliance with tier 1, tier 2 and tier 3 implementation process and required assessments.
8. Facilitate the psychoeducational program and promote the program inside and outside the community.
9. Manage cases to support and monitor participant and family needs, connect with services and provide guidance.
10. Develop a comprehensive academic and social-emotional support plan for students and coordinating with emotional support programs.
11. Write program reports and elevate red flags on compliance.
12. Collaborate and participate in organizational events and develop schedules with practical training sites.
SUPERVISORY RESPONSIBILITIES:
§ Determine schedules, sequences, and assignments for work activities, based on work priority and skill of personnel.
§ Provide guidance and direction to subordinates, including setting performance standards and monitoring performance.
§ Encouraging and building mutual trust, respect, and cooperation among team members.
§ Identify the developmental needs of others, developing formal educational or training initiatives, such as, coaching, mentoring, or otherwise helping others to improve their knowledge or skills.
§ Confer with personnel, to coordinate work activities, resolve employee grievances, or identify and review resources needed.
§ Inspect and monitor work areas, examine tools and equipment, and provide employee safety training to prevent, detect, and correct unsafe conditions or violations of procedures and safety rules
§ Recommend or initiate personnel actions, such as hires, promotions, transfers, discharges, or disciplinary measures.
EXPERIENCE, EDUCATION, SKILLS & KNOWLEDGE REQUIRED:
§ Master’s degree in social work, with specialty and expertise in clinical social work.
§ One (3) or more years of experience working with communities, students or adolescents.
