POSITION SUMMARY
Responsible for the management of the MHC Women’s Health Case Management program function to promote health care delivery across the continuum that will enhance the client’s quality of life, decrease fragmentation of care, while efficiently utilizing resources and containing cost. The process includes screening and case finding, comprehensive multidimensional assessment, care/discharge planning, and connection with available resources, implementation of case plans, and ongoing monitoring and reassessment specific to women’s healthcare. The case plan sets outcome time frames that are appropriate to the individual and her family, and are agreed upon by the client/family and the treatment team. The Women’s Health Case Manager is responsible for overseeing the concurrent clinical reviews of all patients in the obstetric service, especially high-risk OB’s, none-obstetric service outpatient needs, admissions, services rendered, duration of stays in the hospital, and development and implementation of individual case plans. Analytical skills are necessary to independently collect, analyze and interpret date, resolve problems requiring innovative solutions and to negotiate in sensitive situations. The WHCM maintains program compliance with the national standards in accordance with the Joint Commission on Accreditation Healthcare Organization (JCAHO), CMS HIPAA, OSHA and other regulatory agencies. The Case Manager reports directly to the Outpatient Services Program Manager, but maintains ongoing communication and input from the Women’s Health Team members.
PRINCIPAL DUTIES AND RESPONSIBILITES

1. Oversees development and maintenance of a resource data base of Federal, State, community, and institutional resources.
2. Engages in professional development activities to keep abreast of state of the art case management practices and current practices in Obstetrics and General Women’s Health issues.
3. Understands the development needs of all ages of patients throughout the continuum of life from childbearing and beyond, and oversees the application of those developmental needs to client case plans.
4. Monitors the tracking and follow-up of the case plan toward outcome achievement including any abnormal findings (lab, x-ray, etc) in collaboration with the provider team concept.
5. Conduct regular and ongoing prenatal chart reviews to include lab results are reviewed and recorded; ensure quad screens and fetal survey’s are completed that the appropriate times and third trimester labs are completed; review last prenatal visit to ensure any follow-ups are scheduled or completed.
6. Review first prenatal charts prior to visit for surgeries, OB history, past medical history and other pertinent history and record on the prenatal forms; ensure charts contain necessary forms and are complete in conjunction with the Outpatient nurse.
7. Track surgical patients needing Bilateral Tubal Ligations and ensure consents are up to date; ensure that repeat cesarean sections patients have their operative report on the chart.
8. Document Normal Pap results into the Women’s Health Package of RPMS and/or CMET or other appropriate computer package.
9. Notify provider of abnormal PAP and Mammogram results. Refer to facility of higher level of care as directed and document. Verify with provider the plan of care and document the patient’s response/adherence to the established plan of care.
10. Document normal mammogram results in Women’s Health Package of RPMS and/or CMET as appropriate.
11. Use appropriate computer system (i Care/CMET) as available for follow-up, tracking, and verification of breast abnormalities.
12. Assist in the referral process in collaboration with the provider/Case Management team concept.
13. Use policy/procedures to interpret PAP and mammogram reports for an appropriate plan of care.
14. Facilitates discharge planning for patients by identifying then need for discharge planning early in the patients stay and alerting the appropriate persons of potential discharge problems; monitors the discharge planning activities to ensure timely discharge in the collaboration with the medical provider.
15. Participates in program improvement goals such as patient outcomes, cost savings, patient compliance, and return to productivity with staff involvement.


Job Qualifications:


MINIMUM QUALIFICATIONS
Associate or Diploma in Nursing from an Accredited College. Current license as a Registered Nurse in the State of Alaska with Case Management, discharge-planning experience preferred. Must have knowledge of acute and chronic illnesses, specialty clinic procedures, and community health nursing sufficient to provide counseling to staff, individuals and families. It is recommended that the applicant has at least 2 years experience in Women’s Health; as an OB nurse, WHCM, or OB/GYN Outpatient Clinic. Basic Life Support (BLS) certification is required. ACLS, NRP, ALSO and PALS are recommended for specialty. Ability to utilize various computer programs, including RPMS, Microsoft Office and Excel database programs. Must demonstrate a wide theory base in order to interact in an effective manner and physicians, health team members, community agencies, and clients/families with diverse opinions, values and religious and cultural ideals. Must have the ability to work autonomously with little direction and be directly accountable for practice. Demonstrates qualities as time management, verbal and written communication skills, problem solving, decision making, priority setting, work delegation and work organization. Ability to lift up to 70 pounds and long periods of time standing or walking. May be exposed to communicable diseases and hazardous material.
