Waiver Client Referral Specialist

We are seeking an individual with direct access to clients age 55+ and older who are on waiver services looking for housing. Clients must be on either CADI, EW, BI, CAC waiver. The ideal candidate will have a strong background in social work, waiver services, and familiar with assisted living facility's.
Duties
- Help locate and refer clients who are waiver funding approved, who need housing in assisted living facility's environment's
- Coordinate with external agencies and resources to locate clients
Experience
- Previous experience in social work, waiver services
- Knowledge of assisted living facility’s is a plus
- Strong communication skills
Pay will be $7500.00 per a referral long as client is successfully placed into facility

Pay: $1.00 - $7,500.00 per month
Expected hours: 5 – 20 per week
