The ideal candidate will be responsible for providing exceptional care to individuals with varying needs, including those with developmental disabilities of all ages through increased access to integrated life choices including skill building, employment, socialization, community integration, recreation and promoting resilience and healing. This role requires a strong commitment to resident rights, driving success and growth in patient care whiles enhancing skills for positive career growth and a passion for supporting individuals with intellectual or developmental disabilities. Responsibilities: - Provide direct care and support to individuals with developmental disabilities in a residential setting - Assist with daily living activities, including personal hygiene, meal preparation, and medication administration - Implement individualized care plans and behavior management strategies - Promote independence and community integration for individuals with disabilities - Ensure the safety and well-being of residents at all times - Maintain accurate documentation of services provided and any changes in residents' conditions Skills: - Knowledge of social work principles and practices - First aid and CPR certification - Familiarity with resident rights and advocacy - Experience in hospice care or working with individuals with medical needs is a plus - Understanding of behavior management techniques and strategies - Ability to work effectively as part of a team and communicate with individuals from diverse backgrounds - Experience in a nursing home or similar healthcare setting is beneficial - Valid driver's license and reliable transportation for transporting residents to appointments or community activities - Basic knowledge of taking vital signs (e.g., blood pressure, temperature) Note: This job description is not intended to be all-inclusive.
