Position Summary: The position of the Supportive Services Coordinator will advocate for the psychosocial needs of the residents who live at Eaton Senior Communities. The Supportive Services Coordinator will provide oversight for health and wellness programs within Eaton’s organization and wellness consulting services Essential Duties:
Follow policies and procedures for the identification of residents’ social and emotional needs.
Represent the resident services needs of residents and families and participate in developing resident programs.
Establish and maintain a working rapport with department managers, employees and external community health, welfare, and social agencies to ensure that programs are properly maintained to meet the needs of the residents.
Be knowledgeable of and provide information regarding third party reimbursement, i.e., Medicare, Medicaid, and other financial assistance programs available to residents.
Inform and interpret residents, family members, and staff of the residents’ personal and property rights. Maintain the confidentiality of all pertinent resident information to ensure protection of residents’ rights.
Assist with notifications for clinics that may include phone calls, helping complete consent forms, handing out flyers, and checking in with residents as needed.
Monitor and or assist with onsite or offsite clinics to ensure positive clinical experience for providers.
Experience:
Bachelor’s degree in related field of study preferred.
One year’s experience related to the field preferred.
Job Type: Full-time Pay: $21.50 an hour with a 3% increase after 90 days. Benefits:
