How You'll Make a Difference: Arkos Health is seeking a Licensed Social Worker/Case Manager. The position of Licensed Social Worker/Case Manager will report to the Director of Social Work. Provides support regarding social determinates of health to plan members and their families. The Social Services Case Manager provides in-home and telephonic assessment of needs, while continuing to develop, monitor, and evaluate progress, while facilitating interdisciplinary approaches to social needs.
Essential Duties and Responsibilities:
Schedule and complete face to face visits with patients/families/caregivers
Assist and provide resources, information, and referrals to support the care plans
Maintain electronic medical records
Provide holistic, family-centered care across treatment settings to improve the quality of life
Provide ongoing departmental education, in-service training through regular chart audits and monthly metrics
Develop effective working and collaborative relationships with inpatient and outpatient medical services such as hospital social workers and other available community resources, agencies, and services
Support the development and implementation of in-depth SSCM training programs, for ongoing education and development
Maintain Quality Improvement Program; Monitor survey results and follow up required to maintain high quality services
Collaborate with interdisciplinary teams for advisement and creative problem-solving for social needs for high-needs members
Conduct in-depth case staffing and support SSCM team through offered guidance and expertise
Conduct Biopsychosocial assessments and utilize that information to provide higher-level case management and care
Participate in educational opportunities, read professional publications, maintain personal networks and participate in professional organizations in order to best serve the patient and their support networks
Support patient, family, and caregivers with cultural and spiritual sensitivity
Participate in interdisciplinary team (ICT) meeting to establish, review, and revise the patient care plan
Complete assessment for social service needs to address barriers to health care
Qualifications and Skills:
Must have Master’s level of Education
Current BLS/CPR certification required
Arizona Fingerprint Clearance Card required
Current flu shot (or sign waiver/follow mask policy) required
Hep B vax/titer proof (Company does offer/or may sign a declination) required
Current TB test and maintain a negative test annually required
Current active, in good standing AZ license, or in the process of obtaining
