Mariemont Care Center is seeking a dedicated and experienced MDS Coordinator to join our team. The MDS Coordinator will be responsible for overseeing and ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments and submissions in accordance with federal and state guidelines Key Responsibilities:
Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current federal and state rules, regulations, and guidelines.
Monitor and ensure the accuracy of medical records and coding of the MDS.
Coordinate interdisciplinary team meetings and communicate effectively with the team to ensure comprehensive care planning.
Educate and train staff on the MDS process and requirements.
Monitor and track MDS submission dates to ensure timely and accurate submission.
Review and analyze data to identify trends and areas for improvement.
Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
Qualifications:
Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid state license.
Minimum of 2 years of experience in MDS coordination in a long-term care setting.
Comprehensive knowledge of MDS 3.0 and the RAI process.
Strong organizational and communication skills.
Ability to work independently and as part of a team.
Proficient in the use of electronic medical records (EMR) systems.
