RESPONSIBILITIES:
Acts as an advocate and liaison to meet clients’ individual healthcare needs by communicating with providers and collateral contacts and utilizing available resources to coordinate efficient and effective care.
Exhibits and applies clinical understanding of the medical diagnosis(es), case management processes, and psycho-social needs of patients and his/her support system toward the development of an appropriate treatment plan in conjunction with the client’s physician(s) and other pertinent allied health professionals.
Works collaboratively and communicates clearly and professionally with physicians, providers, co-workers, and other members of the health care team in order to carry out the established plan of care.
Demonstrates a thorough understanding of and applies the principles/standards of URAC and NCQA, Medical Services initiatives, and Strategic Business Unit goals.
Participates in QA/QI activities and other additional duties as directed.
Identifies areas for potential cost savings, quality improvement and workflow efficiencies.
Telehealth Home Visits for patients (with or without web enablement and/or homebound) with medical and/or dental care needs.
REQUIREMENTS:
Three (3) years clinical experience, two (2) of which must be in a primary care setting preferred.
Knowledge of case management, managed care, health insurance, and the provider community preferred.
Must be able to effectively use a computer for prolonged periods of time and for most required tasks. Knowledge of MS Word, Excel, and Access preferred.
Bilingual (English/Spanish) preferred.
Must be able to move/lift to 25 lbs.
Must be able to climb stairs and carry needed items for Telehealth Home Visits.
