Navigation Services Specialist

Wage
: $28.77/hour + DOE + DOQ
Schedule: Monday - Friday 8:00am - 4:30pm
Preferred Qualifications:
  • Associate Degree in a field related to social services, public health, data analytics, or project management required. Can substitute years of experience for a degree.
  • Experience building and maintaining relationships with community partners.
  • Proficiency in Microsoft Office suite required.
  • Experience with managing data and data systems preferred.
  • Demonstrated ability to maintain attention to detail, high level of organization and accuracy of work, and problem solving skills required.
  • Two years of social services experience; and/or equivalent combination of education and experience preferred.
  • Must have valid Oregon Drivers license; must have been a licensed driver for at least 2 years; must have a driving record that permits coverage under C@Pā€™s company insurance.
Essential Job Duties Include (But Not Limited To):
  • Oversee the waitlist and intake process for C@P programs by contacting applicants on the waitlist, facilitating transportation, and setting up the intake process with the relevant program for new participants.
  • Partner with the Navigation Services team on property management tasks including keeping tenant tech up to date, posting and mailing notices, and following up with maintenance requests.
  • Coordinate with internal and external partners to facilitate seamless entry into C@P programs.
  • Provide timely response to service inquiries from community partners and applicants. Assist with the referral process to ensure applicants are connected with the programs they are eligible for.
  • Provide coverage at the day center including responding to participant needs and connecting individuals with the resources they are eligible for.
  • Document relevant information in HMIS, Activate Care, and other C@P systems using professional communication practices.
  • Utilize principles of trauma-informed care and outward mindset when performing tasks and working with individuals.
  • Assist with transportation requests as needed.
Work environment: Close quarters with vulnerable people including individuals without stable living situations who may be struggling with mental health and substance use concerns. Indoor/Outdoor work environment with frequent interruptions and demands, and occasional unpredictable behavior. Must be able to push, pull, and/or lift 15 pounds or less.
