Medical Coordinator

The Esperanza Program is designed to provide transitional foster care and reunification services to unaccompanied children (UC). Additionally, the program provides wraparound services that include but are not limited to intensive case management, individual and group counseling, daily schooling, acculturation activities, recreation and leisure time, screening and referrals for trafficking and medical services, legal assistance. The Medical Coordinator is responsible for ensuring the safety, structure, guidance, and care indispensable for the well-being of the unaccompanied children.

LAYC/MMYC’s mission is to empower a diverse population of youth to achieve a successful transition to adulthood, through multicultural, comprehensive, and innovative programs that address youths' social, academic, and career needs. To achieve that mission, LAYC/MMYC provides services and opportunities to approximately 5,000 individuals annually to support academic achievement, promote healthy behaviors, and guide youth toward successful adulthood. LAYC/MMYC also conducts advocacy and public policy work to broaden opportunities for youth.



ESSENTIAL RESPONSIBILITIES

  • Serve as a liaison between the Division of Health for Unaccompanied Children (DHUC), the Office of Refugee Resettlement (ORR), LAYC, and network of healthcare providers, including specialists, emergency care services, mental health practitioners, and dental providers.
  • Coordinate medical, dental, and mental health services to meet the needs of the unaccompanied (UC) children and ensure that all new arrivals receive their initial medical assessments within 48 hours as required by ORR.
  • Data entry of UC medical screenings and follow up.
  • Coordinate UC follow-up and referral services.
  • Coordinate and maintain youths’ vaccination and medical records.
  • Create and manage a medical release plan.
  • Provide primary and emergency care for injuries and illnesses—immediate attention to client service requests. Assess and evaluate patients' needs and responses to care rendered.
  • Maintenance and administration of over the counter and prescription medications and supplies as ordered.
  • Educate and counsel youth concerning their medical status and treatment.
  • Trains staff, youth, and foster parents on medication administration, record keeping, prevention of disease, and other health-related topics.
  • Works with supervisor to create and maintain internal medical policies and procedures as mandated by ORR.
  • Attend agency required trainings.
  • Attend department and program meetings to ensure that up-to-date information is received and/or provided and that changes to policy or practice are implemented and adhered to.
  • Provide overnight on-call coverage as needed.
  • Other task and responsibilities as assigned.


EDUCATION & EXPERIENCE REQUIREMENTS

  • A bachelor’s degree in medical field from an accredited college.


SKILLS & QUALIFICATIONS

  • Must be able to provide proof of Immunization (Varicella, MMR, TDAP, Covid &Flu).
  • Bilingual or fluent/proficient: English and Spanish required.
  • Must pass pre-employment drug test.
  • Strong interpersonal and organizational skills.
  • Knowledge of community and local resources.
  • Complete criminal and child abuse/neglect clearances.
  • Flexible, energetic, positive individual with commitment to youth development in a high quality, respectful environment.
  • Ability to work well and communicate with culturally diverse populations.
PHYSICAL REQUIREMENTS
This is largely a sedentary position that requires the ability to speak, hear, see, and lift small objects up to 10 lbs. May require the ability to travel locally and/or regionally.
