Performs a variety of interdisciplinary assignments which include but are not limited to evaluate/assess health trends and risk factors of an individual/family through data collection and analysis, providing surveillance, tracking, and reporting. Works with specific population groups through health promotion and disease prevention activities, participates in assessing and evaluating health care services to ensure that people are informed of available programs and services, provides health education, care management and primary care to individuals and families who are members of vulnerable populations and high-risk groups. The Health Educator promotes individual and population health by developing and implementing health promotion and education initiatives and health behavior-change programs in a variety of settings for the diverse populations represented in Hall, Merrick and Hamilton counties. These have a focus on strengthening community-based partnerships involving multiple public and private organizations and agencies to address the identified community health priorities in Hall, Merrick and Hamilton counties QUALIFICATIONS · High school diploma or equivalent required · Possession of, or ability to obtain a LEAD risk assessor certification · Associate/bachelor’s degree in health-related field preferred · Bilingual (English/Spanish) preferred. · One year of related experience preferred.
