Intake Coordinator I

Health RIGHT 360 offers residential treatment services designed to serve individuals and families with substance use disorders and co-occurring mental health disorders. Our program helps clients address addiction and related issues, increase skills, manage stressors, connect to supportive service, and build a foundation for lasting recovery. Residential programs run 24/7 and Individual and group therapy, family therapy, individualized case management, treatment planning, drug and alcohol counseling, Medication support services, and crisis intervention.

The Male Community Reentry Program (MCRP) is a residential facility with a capacity of 200 beds, designed for adult males who have less than two years remaining on their prison sentences. Instead of being incarcerated in a state prison, eligible individuals can complete the remainder of their sentences in the community through MCRP. The program offers a variety of rehabilitative services focused on substance use disorder (SUD), mental health care, primary care, employment, education, housing, family reunification, and social support. Treatment for substance use disorders and mental health issues provided by MCRP typically lasts from 6 to 12 months, ensuring comprehensive support for participants.

The Intake Coordinator I conducts the initial evaluation of potential clients to assess their requirements for Substance Use Disorder (SUD) services, completes admission paperwork, and creates the initial Treatment Plan. They ensure a smooth transition for clients to their treatment team, ensuring that new clients are stabilized, and immediate needs are met. The Intake Coordinator I collaborates closely with counselors, case managers, therapists, and other team members to organize and oversee client concerns, make necessary referrals, and offer educational support.

KEY RESPONSIBILITIES
Assessment and Client Care
  • Welcome clients to the program and ensure high-quality customer service is provided.
  • Facilitate admissions and discharges.
  • Coordinate client referrals and verify services as necessary.
  • Conduct thorough initial assessments to determine candidates' needs.
  • Ensure immediate client needs are addressed, and appropriate referrals are made.
  • Assist clients in applying for Medi-Cal and/or County funding if necessary.
  • Develop and implement Initial Treatment Plans as required.
  • Facilitate smooth transitions by providing warm handoffs to residential treatment staff.
  • Monitor client progress and follow up to ensure services are effectively obtained.
  • Provide crisis intervention within the defined scope.
  • Transport clients as assigned.
  • Perform urinalysis for clients and document test results accurately.
  • Facilitate group counseling and carry up to 10 caseloads.
Collaboration
  • Cultivate collaborative relationships with community partners.
  • Collaborate with the Manager I, Admissions for weekly verifications and monthly billing cycles.
  • Communicate effectively within interdisciplinary teams and actively participate in case reviews during team meetings.
  • Communicate and coordinate with external referral agencies to facilitate client placements based on needs and program availability.
  • Ensure clients are authorized for treatment and coordinate with relevant departments such as the Department of Adult Parole Operations and Male Community Reentry Program.
  • Communicate with funding agency to ensure clients are authorized for treatment.
Administrative
  • Document pertinent client information accurately and comprehensively into various electronic systems.
  • Process billing and verification activities accurately and reconcile discrepancies.
  • Write and finalize progress notes within a specified timeframe.
  • Ensure completion of all necessary documentation related to admissions processes.
  • Maintain compliance with relevant registration or certification standards.
  • Uphold internal and external regulatory requirements such as HIPAA, agency policies, and other applicable standards.
Training and Professional Development
  • Attend and actively engage in meetings, training sessions, and supervision.
  • Stay updated with relevant emails and respond promptly as needed.
  • Arrange work schedules in alignment with agency requirements.
  • Complete assigned training promptly to enhance professional competence.
QUALIFICATIONS
Education, Certification, or Licensure
  • High school diploma or equivalent required. Bachelor’s degree in Health and Human Services, Social Work, Psychology, or other related field preferred.
  • Registration or Certification as a Substance Use Disorder Counselor from an accredited California agency (CCAPP, CAADE, CADTP).
  • Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
  • Valid California Driver’s License and access to registered and insured transportation.
Experience
  • Experience with admissions and discharges from a community mental health or substance use disorder treatment setting.
  • Experience in culturally diverse substance abuse populations with co-occurring disorders or with the history of incarceration desired.
Background Check and Other Requirements
  • Qualified candidates with a criminal record will be considered for employment.
  • Must be capable of meeting health screening and tuberculosis testing requirements.
  • Must be capable of maintaining credential requirements.
  • Must be capable of obtaining and maintaining CDCR clearance and requirements.
