Community Health Worker Covington County Part Time
Position Overview: We are seeking a dedicated and compassionate Community Health Worker to join our team in Andalusia. This part-time position will focus on supporting consumers and their families in navigating and accessing comprehensive healthcare services. The role is pivotal in ensuring seamless coordination of care across various providers and services, enhancing the overall healthcare experience for our community members. Qualifications:
Must have a high school diploma (HS) or GED.
Strong understanding of healthcare delivery systems and the importance of integrated care.
Experience or familiarity with care coordination processes, including managing transitions between providers and services.
Proficiency in health IT systems is a plus, particularly as it relates to supporting care coordination activities.
Additional certification in case management or related fields is highly desirable.
Responsibilities:
Coordinate care across multiple healthcare providers and services to ensure that consumers receive comprehensive, continuous care.
Assist consumers and families in understanding and navigating the healthcare system, including the use of health IT systems when appropriate.
Work closely with other healthcare professionals, including primary care providers, mental health professionals, and social services, to create and implement individualized care plans.
Facilitate communication between consumers, families, and healthcare providers to ensure that care is consistent with the consumer’s needs and preferences.
Maintain accurate and up-to-date records of care coordination activities in compliance with healthcare standards and privacy regulations.
Stay informed about best practices in care coordination and participate in relevant training and professional development opportunities.
ALL candidates must be able to type and have basic computer skills, possess a valid AL driver’s license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance with a clear background check.
