Clinical Navigator
Up to $50/hr – Clinical Navigator – Newark, CA – No Weekends!

- Accurately collect and review clinical information to facilitate appropriate patient referrals.
- Serve as the key liaison between referring physicians and the hospital, ensuring clear and effective communication.
- Schedule and coordinate appointments while maintaining strong relationships with referring providers and their office staff.
- Collaborate with internal teams to develop and refine referral workflows, ensuring a seamless experience for patients and providers.
- Navigate patient charts and clinical records to ensure proper documentation and communication of patient needs.
- Support referring physicians with timely updates and facilitate communication with hospital faculty when necessary.
- At least one (1) year of related experience in a healthcare setting.
- High school diploma or GED equivalent.
- Active California LVN license in good standing.
- Proficiency with clinical systems and patient record software.
- Outstanding customer service skills, especially in high-pressure or fast-paced environments.
