Compensation package: · Competitive salary · Employee benefits available include the following: · Health and related insurance after 30 days · Paid Time Off available after 90 days, 21 days per year · 401(k) Plan with a company match The Clinical Case Manager serves as provider for resident-centered services that promote coordinated, effective treatment by facilitating the communication exchange between referral source, physician/treatment team and payor and managing timely transitions through the phases of residential care. Key Responsibilities:
Facilitates communication regarding resident treatment process and needs with referral sources and payor.
Schedules and facilitates of treatment team reviews and documentation.
Ensures coordinated, effective resident care including admission, continued stay, and discharge requirements.
Ensures positive relationships with external and internal customers.
Travels to resident’s community representing the facility at FAPT meetings and court hearings.
Ensures continued availability (re-certification) of financial resources throughout the residents stay.
Other duties as assigned based on the needs of the facility.
Minimum Qualifications:
Bachelor's degree in Human Services, Masters preferred
Two years experience in Behavioral Health
Must have strong customer service skills
Position is subject to a criminal background investigation, drug screen and Tb test
Must be at least 21 years old
