At BCRC our Case Managers play a crucial role in supporting individuals with developmental disabilities and behavioral health needs. This position involves developing and managing programs that enhance the quality of life for our participants to achieve outcomes while ensuring that their needs are met through effective relationship management and administrative oversight. Our Case Managers work collaboratively with participants, staff, families, and other stakeholders to create goal plans and facilitate access to necessary resources. Responsibilities
Develop and implement individualized support plans for participants, focusing on their goals which may include, habilitation, employment or developing life skills to achieve more independence.
Manage relationships with participants, staff and service providers to ensure comprehensive support.
Collaborate with staff involved in program delivery, providing guidance and support as needed.
Conduct assessments to identify participant needs and recommend appropriate services.
Oversee program management activities, ensuring compliance with regulations and best practices.
Maintain accurate documentation of participant interactions, progress, and outcomes.
Collaborate with community resources to facilitate access to services and vocational training.
Oversees participant vocational programming preparing individuals for competitive employment.
Initiates goal / behavior plans and trains participant in new job skills, independence skills, problem solving skills and decision making necessary for competitive employment.
Acts as a liaison and assures a positive relationship with participants and their families, coworkers, outside agencies and community employers.
Maintains participant files and completes all necessary reports in an accurate and timely manner including job analyses, training plans, participant evaluations and other reports as needed to meet state requirements.
Oversees program planning, implementation and adherence to participant support and behavior plans designed to achieve participant outcomes.
Writes job analysis, completes participant assessments and promotes worker independence and self-help skills.
Meets with Program Manager and Employment Specialists as scheduled to plan participant program toward competitive employment.
Transports participants in personal and company vehicles as needed with mileage reimbursement.
Requirements
Bachelor’s degree in Social Work, Psychology, Special Education, Vocational Counseling, Public Health, Criminal Justice or other social service related field
Proven work experience in program / case management within the behavioral health or developmental disabilities sector.
Strong relationship management skills with the ability to work effectively with diverse populations.
Knowledge of relevant regulations and best practices in social work or case management.
Excellent administrative skills, including record keeping and report writing.
Ability to work independently as well as part of a team in a fast-paced environment.
Strong problem-solving skills and the ability to manage multiple priorities effectively.
