Case Manager - Children's (Wraparound Facilitator)
Hegira Health, Inc. (HHI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HHI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages. Hegira Health's Children's Outpatient Services clinic offers quality, comprehensive, compassionate specialty services to children with serious emotional disorders and their families from a team of dedicated professionals. Wraparound Facilitators see strengths in families where others may not and use those strengths to move families forward. Rewarding work where no two days are the same. Wraparound provides an opportunity to make a difference for children through creativity, coordinating with other systems, and working with the entire family. Wraparound requires flexible hours since they meet with families, which means after school and after work hours. They also attend court and school meetings in the mornings when necessary. Wrap Facilitators do flex their hours and most start at 11:00 AM-12:00 PM and work until 7-8:00 PM. If they work a longer day one day, they can flex on another day and end early. Requirements:
Bachelor’s Degree from an accredited institution
Two years of experience in a human services field (in a related position), of which at least one year included providing services to children and families
Have knowledge of wraparound process philosophy
Have working knowledge of community resources
Responsibilities:
Facilitate wraparound process in keeping with the Michigan Wraparound Best Practice Values.
Maintain current knowledge of implementing the Wraparound Process model.
Assessment and Service Plan
Complete strengths-based discovery assessment on all clients/families on caseload within 30 days of initial contact with family.
Acquire and/or create visual aids utilized in assessing child and family strengths and needs.
Prioritize child and family needs based on life domains.
Identify cultural issues regarding the values, preferences, beliefs, culture, and identity of child and family and their community.
Evaluate and assess individual strengths for participation on the child and family teams.
Complete individualized Wraparound Service Plan for each family within 45 days from initial meeting with family.
Develop services to facilitate Wraparound Plan and work with referring agency staff and Community Team in coordinating services/supports tailored to meet the family’s needs as identified by the Child and Family Team.
Using a Person-Centered Planning approach, customize set of strategies, supports, and services to meet the unique needs of each child and family.
Tie goals and strategies of the Wraparound Plan to observable or measurable indicators of success, monitor progress in terms of these indicators, and revise plan accordingly.
Build on and enhance the capabilities, knowledge, skills, and assets of the child and family, their community, and other team members using the Strength Based Perspective.
Identify changing child and family strengths and needs ongoing, consistent with the national wraparound model.
Coordinate individual services as need.
Family and Community Team Meetings
Assist Supervisor in Community Team development, as assigned.
Report in person to the Community Team no less than once a month to present wraparound family barriers for feedback.
Organize, schedule, and facilitate the initial Child and Family Team Meetings
Organize, schedule, and facilitate weekly Child and Family Team meetings.
Ongoing - General
Develop and utilize community resources to meet specified needs as determined by the Child and Family Team.
Work as a liaison to other agencies that are available within the community.
Provide for a flexible work schedule determined by the needs of the family, Child and Family Team, and Community Team.
Complete case record documentation.
Follow directives provided in staff meetings.
Utilize available resources when responding to client and family in crisis situations.
Wraparound is traditionally provided to families in their homes or community; however, telehealth can supplement those approaches. Job Type: Full-time Pay: $48,000.00 - $54,000.00 per year Benefits:
