We are thrilled to announce the availability of 6 positions for committed Enrolled Nurses, spanning both facility-based and community roles. Our focus is on transforming the landscape of TB health care, and your role will be pivotal. Whether you're supporting a healthcare facility or out in the community, your tasks will encompass providing top-notch TB health care, facilitating patient care linkages, offering counselling and support, engaging in active TB case finding, and ensuring retention in care. Duties and Responsibilities include but not limited to:
Offer TB services at designated DOH facilities and/or community settings.
Execute TB screenings, especially within key population groups.
Record presumptive TB cases, ensuring subsequent investigations.
Implement FAST and LF-LAM procedures.
Ensure confirmed TB cases are linked to appropriate care.
Undertake TB contact tracing and support TPT implementation.
Oversee consistent TPT treatments following guidelines.
Confirm HIV testing for TB patients, initiating tests if required.
Coordinate efforts for tracing patients lost to care.
Respond promptly to GXP DS/Rif and Tier.net alerts.
Deliver enhanced adherence counseling and link patients to psychosocial support.
Educate patients on various health topics, including TB, TB/HIV co-infections, and adherence.
Assist with mobile digital X-Ray unit tasks when needed.
Regularly update and review TB identification registers and related platforms, such as EDR.Web and TIER.NET.
Organise patient files, ensuring data accuracy and timely updates.
Join weekly facility data reviews and engage in Continuous Quality Improvement (CQI) activities.
Submit periodic reports, detailing program indicators and progress.
Participate in data cleanup at facilities.
Attend key meetings, from facility staff assemblies to district gatherings and Sukuma Sakhe/War Room discussions.
Support and partake in facility awareness campaigns and retention in care initiatives.
Stay informed on the latest DOH guidelines, policies, and SOPs
Attend relevant training for continuous capacity building.
Requirements:
Enrolled Nurse Certificate with active registration with the South African Nursing Council (SANC).
Possession of a Counselling Certificate.
A minimum of 2 years of experience in a TB/HIV-focused environment.
Demonstrated experience in TB case management.
Prior exposure or work within a primary healthcare setting.
A valid driver's license will be an advantage.
Ability to convey information clearly and empathetically, tailoring messages to various audiences.
Ability to collaborate seamlessly with colleagues and contribute positively to team dynamics.
Skilled in identifying issues and coming up with solutions in a timely and efficient manner.
Resilient in changing environments and able to adjust to new situations or challenges with a positive attitude.
