Kōrero mō te tūranga - About the role
We are welcoming applications from competent and professional Clinical Pharmacists who will be responsible for providing a patient-focused pharmacy service, using best practice guidelines, and achieving optimal outcomes.
The Clinical Pharmacist will work primarily in the Pre-admission Department with a focus on medicine reconciliation and medication optimization. The Clinical Pharmacist will also support medication management of complex patients in a multi-disciplinary team focused on “pre-habilitation” care plans and milestones prior to surgery. Patient education focused on pre-operative medicines management is another key aspect of this position.
You will have the opportunity to:
1. Optimise patients prior to planned surgery
2. Manage pre and post operative medication
3. Work as part of an MDT to prepare patients for surgery
4. Have patients and family/whānau/carers as the primary focus;
5. Be proactive and transparent in the management and delivery of pre-anaesthetic care
6. Optimise continuity of care for patients
7. Empower staff to participate in decision making
8. Promote the most effective use of available resources
9. Have an interdisciplinary approach.
Moū - About you
Are you a qualified Clinical Pharmacist who can practise in New Zealand and who would embrace an exciting opportunity in a beautiful province with fantastic work life balance?
Ideally the candidate will have:
- 2 years Post Graduate Pharmacy experience
- Excellent patient assessment skills
- Excellent communication skills
