Care Manager

Clinical Nurse Manager In A Home Care Agency
Location:Limerick and Clare County
Part-time:potentially Full time
Job Overview:
We are seeking an experienced and highly motivated Clinical Nurse Manager / Care Manager with clinical background to lead and manage the clinical operations of our home care agency. This role is essential in ensuring high standards of care, clinical governance, and regulatory compliance along with the HSE (Health Service Executive) guidelines.
The Clinical Nurse Manager will oversee the delivery of safe, effective, and patient-centred care to clients in their homes and ensure all clinical activities align with the quality standards required in Ireland.
Key Responsibilities:
1. Clinical Governance & Leadership:
- Lead the clinical governance strategy to ensure high-quality care and patient safety.
- Develop and implement clinical policies, procedures, and guidelines in line with HSE standards.
- Provide clinical supervision and support to the nursing and care teams, ensuring all staff are trained, competent, and adhere to best practices.
- Conduct regular clinical audits, risk assessments, and quality assurance checks to maintain high standards of care.
- Work collaboratively with the HSE and regulatory bodies to ensure compliance with all statutory regulations.
2. Client Care Management:
- Oversee and coordinate the care plans for clients, ensuring that they are regularly reviewed and updated in consultation with the client, family, and multidisciplinary team.
- Ensure timely assessments, care planning, and follow-ups for all clients.
- Supervise the care delivery process to ensure client care is personalised, compassionate, and adheres to clinical guidelines.
- Respond to and resolve any clinical concerns or incidents related to client care.
3. Staff Management and Training:
- Provide mentorship, supervision, and leadership to the nursing and home care team.
- Ensure all staff are up to date with their mandatory training, including manual handling, infection control, and safeguarding vulnerable adults.
- Lead the induction and orientation of new clinical staff to ensure they are aware of policies, procedures, and client needs.
4. Clinical Risk and Incident Management:
- Lead investigations into clinical incidents or complaints, ensuring root cause analysis and continuous improvement.
- Ensure the agency’s risk management framework is aligned with national standards, identifying and mitigating risks to client and staff safety.
5. Regulatory Compliance:
- Ensure compliance with all regulations and guidelines set by the HSE and other relevant authorities (e.g., HIQA – Health Information and Quality Authority).
- Maintain up-to-date knowledge of clinical regulations and care standards relevant to home care.
- Ensure all reports, documentation, and audits required by the HSE are completed and submitted in a timely manner.
- Oversee medication management, infection prevention, and safeguarding policies to ensure compliance with national standards.
6. Quality Improvement:
- Identify opportunities for improvement in clinical care delivery and implement quality improvement initiatives.
- Engage with clients, families, and carers to ensure feedback is incorporated into service improvements.
Required Qualifications & Experience:
- Registered General Nurse (RGN) with the Nursing and Midwifery Board of Ireland (NMBI).
- At least 3-5 years of clinical experience, with experience in community nursing or home care management.
- Prior experience in clinical governance, quality assurance, or clinical risk management.
- In-depth knowledge of HSE regulations, HIQA standards, and national guidelines for home care services.
- Strong leadership, communication, and organisational skills.
- A passion for client-centred care and quality improvement.
Desirable Skills:
- Postgraduate qualifications in healthcare management, leadership, or clinical governance.
- Experience in managing clinical audits and risk assessments.
- Strong IT skills and proficiency in healthcare systems for record management and reporting.
Key Requirements for Clinical Governance in Home Care:
1. Care Planning: Requires a clear, comprehensive, and person-centred care plan for each client receiving home care services, regularly reviewed and updated in collaboration with the client and their family.
2. Clinical Governance: Home care agencies must establish a robust clinical governance framework to ensure the safety, quality, and effectiveness of care. This includes leadership in clinical risk management, complaints handling, and ensuring compliance with relevant clinical guidelines and legislation.
3. Safeguarding: Ensure that the staff are trained in safeguarding vulnerable adults and can appropriately respond to incidents of abuse or neglect, with mandatory reporting mechanisms in place.
4. Medication Management: Safe administration and management of medications, following national guidelines and HSE standards for medication policies, particularly in community settings.
5. Risk Assessments: Home care agencies are required to carry out risk assessments for clients, including evaluating the safety of the home environment and care needs.
6. Infection Prevention and Control: Maintain high standards of hygiene and infection prevention and control, particularly in clients' homes, as per national HSE guidelines.
7. Staff Competency & Training: HSE mandates that all home care staff (nurses and carers) have the required qualifications and competencies to deliver safe care, with ongoing professional development and training.
8. Incident Reporting: Agencies must have a clear system for reporting, managing, and learning from incidents or adverse events, with appropriate documentation and follow-up.
