Position Title: Care Coordinator - Nursing Purpose Statement: Care Bridge Home is a professional home care and residential care organization dedicated to providing medical and non-medical support services to seniors and individuals with various medical conditions. We are seeking an experienced Care Coordinator to join our team. The ideal candidate will have proven leadership experience, expertise in nursing, healthcare administration and management, patient care coordination, and training skills, along with excellent communication abilities. This is a hospital-based position, and only candidates with a BSc in Nursing (Kerala Registration) and a minimum of one year of experience are encouraged to apply. Role Description: Patient Care Coordination
Lead and coordinate the Home Care program.
Conduct initial intake from referrals and collaborate with the manager to develop comprehensive assessments and care plans.
Address patient and family complaints and concerns, working to resolve issues with managerial support.
Work effectively with a multidisciplinary team.
Perform other duties as required to fulfil the position's functions.
Ensure high-quality patient care by overseeing daily patient services, appointment scheduling, and follow-up procedures.
Address patient complaints and concerns, working to resolve issues and improve patient satisfaction with managerial support.
Monitor client service quality through daily follow-ups with staff and families.
Assist in recruiting and maintaining a list of care staff for home care services, placing and replacing staff as needed.
Collaborate with the manager on patient service coordination, staff recruitment, and ongoing staff placement.
Training
Collaborate with the manager to recruit and provide induction and ongoing training for caregivers and staff members.
Coordinate and teach the certificate course in collaboration with the Training Manager.
Communication
Efficiently manage inbound and outbound phone calls, handling inquiries related to appointments, services, and general information.
Handle correspondence, including emails and phone calls, and maintain office supplies.
Communicate effectively with clients, family members, healthcare staff, and other stakeholders to facilitate smooth operations.
Act as a liaison between healthcare providers, patients, and external organizations.
Facilitate communication and coordination among different departments to ensure seamless operations.
Reporting and Documentation
Maintain and update client files, office records, and documents, ensuring accuracy and confidentiality in accordance with organizational policies and procedures.
Produce clear and concise reports and documentation, including program reports and monthly reports, ensuring timely submission.
Administration
Schedule appointments, interviews, and maintain accurate records.
Efficiently manage front desk operations.
Oversee daily operations and office management, reporting to the manager regarding office requirements and supplies.
Accounts Management
Oversee and manage office petty cash and daily expenses, monitoring expenses and approving invoices with the manager.
Maintain records of staff and client payments.
Issue invoices, oversee service payments and bills, and remind the manager of payment deadlines.
Ensure accurate maintenance of office accounts using accounting software.
Referral Management
Actively engage in marketing efforts to generate new referrals from various sources.
Actively promote student admissions for the certificate course.
Develop new contacts, partnerships, and programs to increase referrals and improve program outcomes.
Compliance Management
Ensure adherence to organizational policies, the code of conduct, and values, implementing and overseeing compliance effectively.
Skills Requirements:
Excellent written and verbal communication skills in English with a professional and caring disposition.
High level of administrative, organizational, and care coordination skills, with the ability to prioritize, organize, and complete responsibilities timely.
Ability to interact professionally with health professionals, consumers, families, carers, and other stakeholders.
Proficiency in Microsoft Office.
Willingness to undertake ongoing training.
Commitment to providing empathetic support and care to the elderly.
Work Requirements:
Willingness to travel and visit patients’ homes and other places as per the requirements
Must be available locally in Thiruvalla
Available to work from 9 am to 5 pm, including Saturdays, and be reachable 24/7 by phone.
Qualification Requirements:
Bachelor’s degree in Nursing (BSc Nursing)
Minimum of 1+ years of experience in hospitals, healthcare settings, or related industries.
Employment Type:
Contract of 6 months, with possible extension to 3 years depending on the performance.
