Case Manager - Youth

The role of the Youth - Case Manager is to coordinate services and engage community resources that support the multi-barriered youth participating in the Real Me program. The Real Me program works with first- and second-generation newcomer youth, aged 12-24 years old, and considered high-risk for criminal or gang involvement. Through the identity based wraparound intervention model, the Case Manager will incorporate six key services to assist vulnerable newcomer youth and families with diverse and complex settlement needs. These services include tutoring, counselling, employment, prosocial, family support, and mentorship. The Case Manager will also coordinate community resources and work with partners to support newcomer youth in their successful socio-cultural integration. The Case Manager is a specialized position that will work intensively with clients, families, and community partners to promote crime prevention in the City of Calgary.
Responsibilities
1) Facilitation – Direct Service Intervention
· Utilize the hi-fidelity wraparound model and motivational interviewing to work with youth, families and stakeholders to support at-risk newcomer youth and bridge the gap between cultures, identity, post migration trauma, systemic racism, and access to services
· Effectively implement the client intake process
· Co-creating the wraparound plan including measures of success and program exit strategies
· Determining the type of services needed and referring families to appropriate resources
· Providing culturally competent crisis counselling, advice, and coaching to client’s and families as required
· Advocating and providing emotional support to participants and their families
· Co-host family engagement sessions to capture parent feedback and offer training services
2) Administrative
· Maintain accurate and descriptive case notes and feedback records
· Monitor, evaluate, and report progress in relation to team planning
· Collaborate with staff and partners to develop an education framework for newcomer parents to promote crime prevention lessons
3) Community Development
· Engage with ethnocultural groups to enhance awareness of need for natural supports
· Participate in community conferences, focus groups, and committees that address newcomer issues or require a newcomer youth perspective (i.e. youth homelessness, mental health, etc.)
· Reaching out to relevant community resources, centers for collaboration, information sharing, training, and system navigation (i.e. education, legal, health, workplace)
· Providing trainings or workshops for partner agencies upon request
· Participate in the planning and organization of engagement sessions
Qualifications
  • Degree (or pursuing) in Human Services, Justice, Policy, Social Work or another related field and/or 3 years working with at risk youth experience
  • Must be willing to submit to police and child intervention checks; to be eligible for CCIS clearance
  • Strong communication and presentation skills
  • Access to a personal vehicle
  • Experience with provincial legislation regarding children and individuals involved in the justice system is an asset
  • Excellent computer skills
  • Assessment and case management experience is an asset.
  • Systems knowledge - justice, education, health, immigration and or be willing to this knowledge.
  • Fluent in both official languages, or a second non-official language is an asset

Working conditions
· Outreach
· Work hours flexible to meet youth and families; may include some evenings and weekends.
· If using vehicle, must provide proof of insurance up to $1 million in liability; up to $2 million to carry clients

Pay: $55,000.00-$60,000.00 per year
