About the Role:
The Northern Adelaide Local Health Network is seeking a Transition Care Program (TCP) Coordinator to join the Aged Care, Rehabilitation and Palliative Care Division.
As a TCP Coordinator, you will play a vital role in supporting patients and their families during the critical transition period between hospital and home or residential aged care. Your responsibilities will include coordinating and facilitating the efficient discharge of eligible clients, ensuring they receive the necessary support and services to continue their recovery journey in a comfortable and familiar environment.

To excel in this role, you will possess exceptional communication and organisational skills, enabling you to collaborate effectively with multidisciplinary teams, clients, and their families. Your empathy and ability to navigate complex situations with sensitivity will be invaluable in ensuring a seamless transition experience for all involved.

In addition to your clinical expertise, you will have the opportunity to contribute to the continuous improvement of the Transition Care Program, ensuring it remains responsive to the evolving needs of our community.

To be eligible for this role you must hold an appropriate degree or equivalent qualification for one of the following Allied Health disciplines working in the service, specifically:
  • Appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia.
  • Appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.
  • Appropriate Degree or equivalent qualification which gives eligibility for full membership of Speech Pathology Australia.
  • Appropriate Degree or equivalent qualification, usually in human movements, inclusive of the exercise physiology elective and eligible for accreditation from Exercise Sports Science Australia (ESSA).
  • Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
  • Appropriate Degree or equivalent qualification which entitles registration with the Pharmacy Board of Australia.
