About the role
Grafton Community Health is seeking a Social Worker level 3 Specialist Palliative Care to work as part of a multidisciplinary team providing inpatient and community social work services to clients, families and carers of the specialist palliative care service. Working within the model of palliative care to identify and address psychosocial needs, provide therapeutic services to clients and provide specialist consultation and other team members in the management of complex cases.
