The need-to-know…
As Rise’s newest Recover Worker - Outreach, (known as the Outreach Support Worker at Rise), you'll play a key role in empowering individuals with lived experience of mental health challenges to build skills and achieve their personal goals. You'll walk alongside clients on their recovery journey, delivering a community-based, person-centred service grounded in a recovery model. You'll also collaborate with adult mental health services to respond to and manage crises, while keeping everyone updated with regular reports on clients' progress, achievements, and any concerns.
A typical day might look like…
  • Work alongside people you support to achieve goals and aspirations they have identified in their individualised plan.
  • Support people using a combination of person centred, strengths based, recovery orientated approaches in response to their needs.
  • Provide regular reports on the person’s progress, achievements and concerns.
  • Support the person and/or representative to attend meetings and appointments that support the goals set out in the individual’s plan.
  • Respond to and manage crisis in collaboration with relevant adult mental health services.
  • Support with the induction of new workers through buddy shifts and sharing of information.
This role is full-time, working across Monday to Friday in our head office (Middle Swan) & other metro locations as required.
The need-to-have…

  • Certificate IV in Mental Health or a related qualification.
  • Proven experience working within a Mental Health Recovery model.
  • Hands-on expertise in creating, delivering, and evaluating plans that empower individuals to achieve their goals and lead fulfilling lives.
  • Strong decision-making skills, using initiative, discernment, and sound judgement, while upholding professional boundaries.
  • Excellent communication skills, both written and verbal, adaptable to a variety of settings.

Appointments to this position will be subject to a NDIS Worker Screening Check / NDIS Worker Orientation Module / Working with Children Check / requires a driving licence and reliable vehicle to use for work purposes.
Costs for new checks will be covered and/or reimbursed by Rise.
