Skilled Nursing Facility LPN/RN

Job description
Canterbury Woods’ Lifecare Community is looking for an LPN or RN to join our team on our fifty (50) bed person-centered care CARF accredited Skilled Nursing unit.
Open Position:
Part Time Every Other Weekend: 7am - 3pm
Job tasks include:

Participate in the development and implementation of care plans, including quarterly reviews, as they relate to care.
Prepare, administer and document medications as per procedure.
Demonstrate to residents and staff personnel, as necessary, the procedures involved in the resident care.
Encourage residents to perform or participate in ADL functions and range of motion exercises. Assists residents with range of motion as necessary.
Collect data and report resident concerns to RN/MD or interdisciplinary team as applicable for follow-through. Assure directions and follow-through on unit and completion of documentation.
Conduct regular walking rounds of nursing unit.

LPN/RN Qualifications:

Must possess a current, unencumbered, active license to practice as an LPN or RN in New York State or permit.
Training in rehabilitative and restorative nursing practices is preferred.

LPN Wage Range: $27.00 - $30.00
RN Wage Range: $32.00 - $38.00
