DEFINITION: This is a management/supervisory position who, under the direction of the President, provides the day to day management and operations for Continuum Care, Inc. (CCI).
QUALIFICATIONS: The Director shall be a Registered Nurse licensed in the VI. Minimum of 3 years in community care and/or hospice. Minimum 1-year supervisory experience. Adherence to the Standards of Practice as established by the National Hospice and Palliative Care Organization (NHPCO). Excellent interpersonal communication skills - written and verbal. Computer competence. Demonstrates a consistently positive attitude in the position, even during stressful times. Ability to multi-task several complicated responsibilities at one time. Ability to effectively manage, supervise, and direct personnel of various abilities.
RESPONSIBILITIES AND FUNCTIONS
1. 24/7 responsibility for the day to day management and operations of the hospice program.
2. Supervision of office personnel and all clinical staff including but not limited to the Interdisciplinary Team, field staff, and volunteers.
3. Ensure compliance with Federal and Territorial laws, including but not, limited to the Medicare Conditions of participation.
4. Work collaboratively and cooperatively with associate RN Administrator to ensure smooth operations and compliance with the Medicare Conditions of Participation.
5. Responsible for the Quality Assessment and Performance Improvement (QAPI) Program including quarterly reports and annual reports.
6. Responsible for the hiring and orientation of all new staff and volunteers.
7. Maintain regular communication with physicians. Meets with top referring physicians at least twice annually.
8. Meet with new physicians to introduce CCI within 60 days.
9. Conduct regular rounds with hospital discharge planners and utilization review coordinators. Visits hospital at least once weekly .
10. Conduct Interdisciplinary Group Meetings as per the Medicare Conditions of Participation.
11. Maintain competence of Electronic Medical Record (EMR) to ensure organizational compliance with Medicare Conditions of Participation and CCI Policies.
12. Ensure the timely submission of clinical notes and all patient documentation, including physician signatures as required .
13. Review all clinical referrals for appropriateness of life limiting illness and adherence to guidelines for prognosis as per the Local Coverage Determination (LCD).
14. Review accuracy and completeness of all new patient certifications /admissions and re-certifications within established timelines.
15. Maintain a schedule of all patient visits by discipline that is updated/verified on a daily basis.
16. Ensure prior authorization of patient insurance benefits.
17. Conduct employee appraisals including introductory and annual evaluations. Evaluations completed within 5 days of the due date.
18. Provide follow-up and root cause analysis on risk management reports. Included as pa1t of QAPI. Reports completed monthly.
19. As per CCI policy and procedure, insure home supervisory visits to evaluate the performance of all field personnel at least annually. Report is summarized on performance evaluation.
20. Conduct disciplinary action of staff and /or volunteers when needed.
21. Maintain monthly statistics no later than five business days after the month closes.
22. Provide regular monthly continuing education for certified home health aides in accordance with federal regulations. Maintains files.
23. Responsible for on-call and after-hours availability.
24. Conduct monthly clinical records review of expired/discharged records for the previous month. Audit for completeness.
25. Ensure timely completion and submission of documents required for physician approval and signature including Plans of Care, Physicians orders, and medication orders.
26. Anticipate equipment and supply needs. Submit requests for authorization to purchase when needed .
27. Participate in annual budget preparation as needed.
28. Maintain optimal level of medical supplies. Makes recommendations for replacement of supplies as needed.
29. Attendance at Administrative Meetings as requested.
30. Demonstrate the ability to conduct business and personnel issues in a consistent and professional manner.
31. Initiate the development or revision of policies and procedures as needed.
32. Demonstrate professional and continuing education at least 6 each 12 months.
