RN Case Manager - $2,506/Week(40 Hrs)-MS
Job Summary
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, collaborating with healthcare professionals, and ensuring optimal patient outcomes.
Responsibilities
Title : RN Case Manager
Contract : 13 weeks (High Possibility of extension)
Shift : Days, 5x8hr
Guaranteed Hours/week : 38
Gross Pay : $2,506/Wk
Location : Jackson, MS
Requirements :
MUST HAVE EXPERIENCE:
Must be RN Case manager with at least 2 years Acute Care Management experience in discharge planning experience.
Experience must include at least 6 mos at a level 1 or large academic Medical Center hospital working with a variety of inpatient units for medical discharge
EPIC CHARTING experience is REQUIRED
5x8hr shifts - non-negotiable
General Certification Requirements (BLS, etc.) BLS, RN Case Manager
Other Certifications/Specialty certifications requirements? CCM preferred but not required
Prefer RN, BSN, *Would consider ADN .
Department: CM - General Case Manager/Discharge planning
Case load/# patients per day/consults per day 15-20
EMR - Will use navi Health to send referrals and plan to switch to EPIC Care Management over the next year)
Do you require Inter Qual criteria or Milliman experience - No, but need to have general understanding of discharge readiness
Is this position for UR, discharge planning, or both? Case Management - Discharge planning
What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.) Will cover all Inpatient units throughout the hospital

- Maintain accurate medical records and documentation
