Responsibilities

• Provide direct patient care, administering medications and treatments as prescribed.
• Assess patients' conditions, including vital signs, and ensure timely documentation.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.
• Monitor patients' progress and make necessary adjustments to the care plan.
• Educate and support patients and their families throughout the recovery process.
• Ensure an accurate and up-to-date electronic health record for each patient.
• Adhere to infection control protocols and maintain a safe and clean environment.

• Valid Registered Nurse license in CA
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of 1 years of experience in subacute or a related setting.
• Excellent clinical skills and knowledge of subacute care.
• Strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced, team-oriented environment.
• Commitment to providing compassionate and patient-centered care.
• Willingness to learn and adapt to new technologies and best practices.
