We are seeking a highly skilled and compassionate Registered Nurse to join our team in the role of Subacute Registered Nurse. In this position, you will provide exceptional care to patients requiring subacute care, ensuring their comfort and well-being throughout their stay. You will have the opportunity to work with a dedicated team of healthcare professionals in a state-of-the-art facility. As a Subacute Registered Nurse, you will be responsible for delivering direct patient care, assessing patients' needs, and developing and implementing individualized care plans. You will collaborate with other healthcare professionals to ensure the highest quality of care, and you will play a crucial role in patient education and support during their recovery process.
Responsibilities
Provide direct patient care, administering medications and treatments as prescribed.
Assess patients' conditions, including vital signs, and ensure timely documentation.
Collaborate with the interdisciplinary team to develop and implement individualized care plans.
Monitor patients' progress and make necessary adjustments to the care plan.
Educate and support patients and their families throughout the recovery process.
Ensure an accurate and up-to-date electronic health record for each patient.
Adhere to infection control protocols and maintain a safe and clean environment.
Requirements
Valid Registered Nurse license in CA
Bachelor of Science in Nursing (BSN) preferred.
Minimum of 1 years of experience in subacute or a related setting.
Excellent clinical skills and knowledge of subacute care.
Strong communication and interpersonal skills.
Ability to work effectively in a fast-paced, team-oriented environment.
