Registered Nurse (Disabled/LTC) - Santa Fe, NM Shifts: Days (Mon-Fri) The usual shift is 8-4 and there could be on call hours required Job Description: We are seeking a skilled and empathetic Registered Nurse with experience in disability care to join our team. The ideal candidate will have expertise in assessing, planning, and delivering care to individuals with physical, intellectual, and developmental disabilities. This role involves collaboration with a multidisciplinary team to address the unique healthcare needs of our patients. Roles and Responsibilities:
Conduct comprehensive health assessments tailored to individuals with disabilities living in Group homes in the Santa Fe area
Will be required to do staff trainings, etc. Each group home has a max of 4 patients and They would hold a caseload of about 18 patients which are of the older demographic.
Administer medications, treatments, and interventions in line with care plans.
Monitor and manage chronic conditions commonly associated with disabilities.
Educate patients, families, and caregivers on managing health conditions and accessing resources.
Qualification/Experience:
Associate or Bachelor's Degree in Nursing
Experience working with persons with developmental disabilities is required
Licenses/Certifications:
Must be a licensed RN in the state of New Mexico
Current valid driver's license
