The Gathering Place (Brattleboro Area Adult Day Services), is an Adult Day program serving Windham County Vermont. We serve adults with a wide range of physical and cognitive abilities. Our participants enjoy music, arts, games, exercise and companionship - just to name a few! We are seeking a community-minded Registered Nurse to provide oversight, integration and coordination of comprehensive health services and rehabilitative programs that meet the needs of The Gathering Place program participants. Primary duties include:
Completing health assessments for all participants.
Outreach to participant care team members (primary care providers, case managers, caregivers, etc) to obtain, review and document clinical information to ensure a compliant and complete participant record.
Provides caregiver support and assists participants/caregivers in accessing and utilizing community resources.
Development, maintenance and documentation of individual person-centered care plans in collaboration with participants, families and/or program staff.
Routine and preventive nursing care, including: health monitoring; medication administration; medical crisis management; communication with physicians and families; vital sign checks; health nutrition and medication education
Provide instruction and support to direct care staff regarding each participant's health care and nutritional needs; provide annual competency verification and delegate nursing tasks as appropriate.
Completes appropriate forms, documentation and reporting to ensure record compliance with Vermont Adult Day Standards.
Schedule: Monday-Friday, days (regular business hours). No weekends or evenings; we are closed major holidays. Full time preferred, part-time considered for the right candidate. Rate of pay: From $36/hr depending on experience; please include desired pay rate in application materials Benefits include up to 15 days paid time off in the first year (accrued based on hours worked) plus 7 paid holidays and dental, vision, life, LTD and AD&D coverage at no cost (employee only). 403(b) match after one year. Qualifications
Nursing assessment skills
Effective written and verbal communication
Excellent documentation skills with experience working on electronic health records
Good organizational and leadership skills
Computer competence; familiarity with Word and Excel
Education, Experience, Certifications
Registered Nurse with current Vermont State license or eligible to practice in the state of Vermont via a multi-state license
At least one year’s nursing experience, preferable working with older adults and adults with disabilities and their families
