Olmstead Housing Program Independent Living Specialist
WILC is a consumer-controlled nonresidential center for independent living that provides information and advocacy to persons with disabilities. The goal of this Program is to assist with the development of home and community-based services and supports needed by individuals and family members with a disability. POSITION: Olmstead Housing Program Independent Living Specialist (OH ILS) HOURS: Full time, 35 hours per week, with benefits RESPONSIBILITIES: 1. WILC OH ILS will provide linkage and referrals and work with participants to identify goals for independent living that will be reviewed semi-annually with each participant throughout their participation in the program. 2. WILC OH ILS will accept referrals from, hospitals, Managed Care Organization (MCO), medical respite, Performing Provider System (PPS), or Skilled Nursing Facility (SNF). and from NYAIL. Track referrals and caseloads using NYAIL and CIL Suite databases. 3. OHS ILS will utilize the database created by NYAIL to provide information about contacts, participant needs, and services provided. ILC will have technology available to OH staff to complete web-based reporting in a database. 4. The assigned ILS will work with participants to identify goals and community living linkages to resources needed once discharged. The ILS will also work with participants to construct an individualized service plan to reflect goals, support needs, and strengths. The ISP will be maintained over the course of program participation by the ILS and participant to ensure needs are being met and goals are in line with participant desires. 5. ILS staff funded through this program will avail themselves of training developed by NYAIL. ILC will identify the training needs of its OHS staff, so that NYAIL can incorporate the information in scheduled training. 6. ILS will complete all reporting requirements for OHS, as developed by NYAIL and WILC. This includes but is not limited to case notes, timely submission of invoices, requests for tenancy approvals and community transition services requests, and quarterly reports. 7. ILS will document and share with NYAIL barriers that hinder stable housing and/or goal-related issues. With NYAIL and other providers, ILS will suggest solutions to NYAIL that will be provided to the New York State Department of Health to address barriers. 8. ILC will help identify strategies and goals for local outreach, use outreach materials provided by NYAIL, and identify partnerships to be developed locally. 9. WILC OH staff will ensure all staff entering areas where patients/clients/residents/consumers might be present, including institutions, clinics, and homes, will either be vaccinated for the current influenza season or will have on file a signed agreement that the staff will wear a facemask. ILC will maintain a record of proof of vaccination for each staff person and provide a copy of this documentation to NYAIL. QUALIFICATIONS: · Bachelor's Degree or equivalent experience · Demonstrated abilities in written and oral communication skills · Ability to use word processing computer programs · Reliable transportation, valid driver’s license and insurance coverage · Experience working with people with disabilities a plus · Knowledge of Housing/Rental Options Job Type: Full-time Expected hours: 35 per week Benefits:
