Job Summary
The Memory Support Coordinator maintains the Memory Support Program by planning and coordinating the implementation of a full recreational activity program for all residents in the program. This position provides Dementia Training to staff members working within the Memory Care neighborhood and other Neighborhoods within Barclay Friends. Provides support services to residents as needed.
Essential Function: 1. Effectively plans and implement programming for the Memory Support Neighborhood by working the Recreation Director to plan and structure a daily program for each resident in the Memory Support neighborhood. The Memory Support Coordinator will organize and direct activities in accordance with residents’ needs, interests and capabilities; including both group and one-on-one activities. This position will demonstrate creativity in the development of daily recreational program for residents with cognitive defects and oversees all staff members assisting with the program. This position will purchase materials and supplies for the program, as needed, in addition to ensuring payment vouchers are completed and submitted to the Recreation Director in a timely fashion. The Memory Support Coordinator will ensure all operations of the Memory Support Program are working effectively and will make the Recreation Director aware of any issues or broken equipment. 2. The Memory Support Coordinator will effectively communicate with coworkers and management to share information related to resident needs, progress and interests. This position will provide information and ideas to Recreation Director to improve recreation in Preston. This position will also demonstrate good customer service when working with residents, family members and outside vendors. The Memory Support Coordinator will serve on Barclay Friends Committees, as assigned. 3. Evaluates the overall effectiveness of the Memory Support Program by monitoring participation and resident feedback. This position will offer suggestions for new programs and set goals/objectives to improve the overall program to Recreation Director. The Memory Support Coordinator maintains resident participation results by retaining individual files on each resident via AOD and recording reactions and capacity to participate on an activity tracker. In addition, this position will work with appropriate staff members regarding writing resident assessments, updating progress notes and recording resident attendance. The Memory Support Coordinator will participate in IDT care plan meetings as needed. 4. Assists the Recreation department in other tasks, such as transporting residents safely and efficiently to and from programs, activities and events. The Memory Support Coordinator is responsible for helping residents get to their desired activities in accordance with the residents’ needs, interests and capabilities, as well as for loading and unloading residents into and out of the Barclay Friends van. This position must always oversee the safety of the residents, including when on trips outside the facility. This position is required to drive the van, while abiding by the Department of Motor Vehicle rules, as needed. This position participates in the daily life of the Memory Support Neighborhood by providing support during resident meals and by developing therapeutic interventions when requested by the Personal care staff. Experience: Minimum two years of experience working with older adults with dementia. Must demonstrate patience to work with residents at all cognitive levels. Must display a keen interest in research related to dementia and be willing to learn and implement best practices in the field of dementia care. Must be familiar with assessment and care planning process. Must be computer literate and must have a current Pennsylvania driver’s license. Must be willing to learn the state of Pennsylvania's Personal Care regulations. Must pass Direct Care Training course and have current CPR and First Aid Certification. Degree required. Certifications obtained from National Certification Council for Activity Professionals (NCCAP) and/or National Council of Certified Dementia Practitioners or similar field preferred but not required. Successfully complete the Department of Human Service’s Direct Care Training Course and competency test.
