MDS Nurse
We are seeking an experienced MDS Nurse to join our healthcare team. The MDS Nurse plays a crucial role in ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments for our patients.
Duties
- Conduct comprehensive assessments of patients' health status
- Collaborate with interdisciplinary teams to develop individualized care plans
- Ensure compliance with regulatory requirements and quality standards
- Monitor patient progress and update care plans as needed
- Utilize electronic health record systems such as Epic for documentation
- Coordinate with healthcare providers to optimize patient outcomes
- Provide guidance on discharge planning and post-acute care needs
Experience
- Prior experience in pediatrics, case management, primary care, DRG, PICU, acute care, and discharge planning is preferred
- Familiarity with Epic or other electronic health record systems is a plus
- Experience in home care or inpatient settings is beneficial
Job Type: Full-time
Pay: From $45.10 per hour
