Nurse - LPN Overview:
Plan, deliver, and monitor nursing care for our residents, ensuring their well-being and comfort are prioritized at all times.
Conduct comprehensive clinical assessments of residents' health status, documenting findings accurately and promptly.
Monitor and manage the administration of medications, as well as the status of chronic illnesses, to optimize residents' health outcomes.
Collaborate effectively with other departments, external providers, staff, and families to coordinate personalized care plans tailored to each resident's unique needs.
Keep residents' families and involved parties informed of any changes in health or care plans, fostering open communication and trust.
Provide exceptional customer service to residents and families, promoting a warm, welcoming atmosphere that enhances their overall experience.
Respond promptly to any emergency needs of residents, taking decisive action and documenting follow-up procedures appropriately.
Nurse - LPN Qualifications:
Valid Licensed Practical Nurse (LPN) license in the state of Minnesota.
Previous experience in nursing care, particularly in geriatric care settings, is highly desirable.
Demonstrated ability to work autonomously while maintaining high standards of care and professionalism.
Excellent organizational, planning, and communication skills, with a strong attention to detail.
COVID-19 vaccination or documented exemption required.
