Virtual Care Navigator Opportunity Description
Our client is a young start up organization that combines technology with the power of human support to help families navigate the challenges of aging and caregiving. They have a current opening for a Virtual Care Navigator that has experience working with geriatrics or eldercare.
Virtual Care Navigator Requirements:
The compensation scale listed is a general salary range and is not intended to represent the actual annual or hourly compensation rate for this role. An offer will be made to the selected candidate at the conclusion of the interview process based on their relevant experience and the responsibilities of the position. Our client does offer a competitive compensation package potentially including incentives, health and dental insurance, PTO, retirement plans (401K) and opportunities for professional development.

Virtual Care Navigator Opportunity Description
Our client is a young start up organization that combines technology with the power of human support to help families navigate the challenges of aging and caregiving. They have a current opening for a Virtual Care Navigator that has experience working with geriatrics or eldercare.
Virtual Care Navigator Requirements:
- At least 3-5 years experience as a clinical social worker or another professional specializing in geriatrics or eldercare.
- Master Level Clinical Social Worker, Geriatric Certification preferred
- Knowledge of the aging process, conditions affecting older adults, and care management best practices.
- Case management experience and medicare understanding
- Be comfortable with technology including CRM tools, Google Suite, and Zoom.
- Provide education and advocacy and offer counseling and support
- Identify care problems and offer solutions
- Act as a family caregiver coach and create a care plan
- Participate in sales and marketing initiatives including leading webinars
