Dementia Program Coordinator

HOV’s innovative and award-winning Supportive Care for Dementia Program is at the forefront of providing unparalleled support and education to care partners and those with dementia as they navigate the journey, from the early stages of diagnosis to advanced care needs. This is a non-hospice program that is anchored at the state-of-the-art Dementia Care and Education Campus (www.dementiacampus.org) established in Spring 2022, this program offers a unique opportunity to collaborate with leading experts in the field and be a pivotal part in our mission to help those in our community live well with dementia.

Position Profile
The Dementia Program Coordinator will help in the day-to-day support for the Supportive Care for Dementia Program such as scheduling, customer service focused communication, data creation and tracking as well as team-oriented support.
Responsibilities
  • Provides administrative and phone support for the Supportive Care for Dementia Program.
  • Performs staff assignments and scheduling.
  • Performs accurate and timely data entry.
  • Provides administrative support for Contracts and Grants.
  • Maintains and enhances professional skills.
  • Adheres to high standards of personal and professional conduct.
Minimum Qualifications
  • High school diploma or equivalent experience.
  • Minimum of 1-3 years’ experience in an administrative support role in healthcare, education, or quality improvement background is strongly preferred.
  • Previous administrative/project support experience.
  • Excellent communication and interpersonal skills.
  • Proficient computer skills.
