Community Outreach and Respite Care Coordinator

POSITION SUMMARY

The Community Outreach and Respite Care Coordinator will work closely with families, care providers, and community resources to arrange temporary relief for caregivers, ensuring that their loved ones receive safe and high-quality care during their absence. This role involves assessing client needs, scheduling care services, and providing support and guidance to both caregivers and respite care providers.


ESSENTIAL RESPONSIBILITIES

  • Promote respite care services within the community through presentations, workshops, and partnerships with local organizations, while increasing awareness of the importance of caregiver respite and the resources available Conduct assessments to understand the needs of both caregivers and their loved ones
  • Determine the level of care required during the respite period and match clients with appropriate care providers
  • Schedule and arrange respite care services, including in-home care, adult day programs, or short-term stays at care facilities, ensuring that all services are delivered according to the care plan and meet the client’s needs
  • Build and maintain relationships with respite care providers, including home health aides, nurses, and care facilities and ensure that providers are trained, qualified, and prepared to meet the specific needs of clients
  • Offer guidance and support to caregivers in planning and utilizing respite care
  • Provide information on available resources, financial assistance, and strategies for managing caregiver stress
  • Regularly check in with caregivers and care providers to assess the quality of care and address any concerns to include adjusting care plans as needed based on feedback and changing needs
  • Maintain accurate records of client assessments, care plans, service schedules, and provider interactions and ensure compliance with all relevant regulations and organizational policies
  • Respond to emergencies or urgent situations where immediate respite care is needed and quickly coordinate services to ensure that caregivers can take necessary breaks without compromising their loved one’s care


WORK HOURS, SCHEDULE & FLEXIBILITY
This position primarily works traditional office hours to meet the needs of the agency. Some evening or weekend hours may be required to support the agency.

  • Ability to face adversity and help to de-escalate individuals who become upset regarding ineligibility for assistance or other matters
  • Utilize strong analytical skills with the ability to recognize and understand complex issues while being able to circumvent and resolve those issues professionally; demonstrates flexibility and adaption when complex issue arise
  • Build relationships and collaborate with community partners and resources
  • Demonstrate cultural competency by communicating and working effectively with those from diverse backgrounds and creating and sustaining an environment in which people from diverse backgrounds and perspectives can succeed


REQUIREMENTS
  • Bachelor's degree in social work, nursing, gerontology or a related field
  • Five years' experience in caregiving, respite care management or case management
  • Strong understanding of aging and disability issues, as well as the challenges faced by caregivers
  • Ability to pass a background check
  • Valid Utah driver’s license
  • Reliable transportation
  • Proof of current automobile insurance
  • Ability to become insured under JFS upon hire
  • Compassionate and empathetic approach to supporting caregivers and care recipients
  • Proficiency in Microsoft Office and electronic health record systems
  • Positive attitude, proactive mentality, responsive, and strong work ethic
  • Must have effective verbal and written communication skills
  • Must have excellent organizational skills
