Builds trusting relationship with patients to facilitate access to health care services and meet health needs
Engages patients to participate in health promotion, maintenance and restoration activities
Works in partnership with our multidisciplinary team (including providers, nursing, CHWs, care coordinators and others) to advocate for patients and ensure health needs are met
Assists patients and families with accessing, navigating, and connecting with internal providers and services, outside community resources and programs, and other services as needed
Revises and reprioritizes plan of patient care, as needed; identifies and addresses any barriers effecting a patient’s ability to adhere to plan
Conducts community outreach, home visits, shelter/street outreach to identify people who are experiencing homelessness and other social support needs and educates about health needs
Requirements:
Graduated from an accredited two year nursing school
Must possess a current NYS Professional Registered Nurse license
Must have current CPR certification
The ideal candidate has knowledge of case/care management principles and has the ability to work with a diverse group of patients and understands the impact of social determinants on health

Hourly rate: $30 - $40 an hour (based on a full-time, 40 hour work week). Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related factors. RN Tuition Reimbursement Potential Our FQHC status provides a unique opportunity for tuition reimbursement for registered nurses, up to 60% repayment of qualified student loans directly related to a nursing degree. This tuition reimbursement program requires application and is not guaranteed. RNs who are granted reimbursement are required to fulfill a two year, full time commitment to maintain eligibility. Additionally, there is potential for an additional 25% repayment for a third year commitment. This position offers a sign-on bonus for external hires, paid in installment's throughout the employee's successful first year of employment. Why Neighborhood?
