REWARDING OPPORTUNITIES: Serve as the primary point of contact for SCBDD youth ages 3 through graduation, which includes working collaboratively with the school districts, families and/or caregivers of eligible youth. Provide information, resources, referrals, and offer support to youth, their families and caregivers, and support teams. Provide case management services to assess and identify needs and coordinate resources, referrals, assistance, and advocacy supports as needed. Assist families through navigation of resources during crisis situations and/or events requiring services to keep the family together and safe. Serve as a support liaison for the school district and assist them in facilitating eligibility referrals and connecting with resources. Facilitate Family Support Service applications. Initiate referrals to the Service and Support Administration department or other community resources as appropriate. Perform other duties as assigned by the Director of Youth and Provider Services. Maintain a positive attitude towards ensuring excellent customer experiences with all interactions. QUALIFICATIONS: Completion of a Bachelor’s degree in rehabilitation, human services, social work, special education, or related field. Two years of professional experience working with individuals with developmental disabilities preferred. Must have a valid Ohio Driver’s license and be able to pass a physical examination and have an acceptable criminal background check (ie, Bureau of Criminal Investigation).
