Hours:
8 hours per day / 5 days per week
Responsibilities:
1. Conduct oneself professionally in all aspects of the job.
2. Support the agency's philosophy and objectives.
3. Uphold policies regarding professional ethics.
4. Inform patients about additional services offered by the organization.
5. Attend in-service trainings and educational workshops.
6. Participate in all required staff meetings.
7. Collaborate and communicate effectively with team members.
8. Develop comprehensive Individualized Treatment Plans for patients.
9. Assist with employment and vocational referrals.
10. Provide outreach and follow-up services to patients.
11. Facilitate discharge planning for patients.
12. Engage with external entities to assist patients with various needs.
13. Maintain detailed progress notes in patient records.
14. Perform other duties as assigned by the Director or Administrator.
Qualifications:
For AOD Programs:
- Enrollment in an academic Addiction Studies program and registration with CCAPP, CAADE, or CADTP.
- 1 year of experience in a Behavioral Healthcare setting.
For all other Programs:
- 1 year of experience in a Behavioral Healthcare setting (HIV programs) and DVSS Certification for DV specific staff.
- Bilingual English/Spanish preferred.
- Valid California driver’s license and insurance.
