JOB TITLE: CASE MANAGER- COORDINATED ENTRY (CCN) DEPARTMENT: Coordinated Entry REPORTS TO: Director of Coordinated Entry POSITION SUMMARY: The Coordinated Entry Case Manager is a full-time position designed to provide Case Management support to each client in the assigned caseload. The Case Manager assesses the client’s Case Management needs, develops the client’s Case Plan, implements the Case Plan and documents progress. The Case Manager also assists the client with obtaining needed services, benefits, and housing via advocacy, resource linkages and referrals. ESSENTIAL DUTIES AND RESPONSIBILITIES: Program Duties
Receive and audits assigned files for accuracy and completeness.
Document changes to client’s eligibility status and notify Case Management Supervisor and
Operations Coordinator.
Develop and maintain relationships with partner agencies, community organizations, public assistance entities, and stakeholders to facilitate successful referrals, service delivery and continuity of care.
Develop and maintain relationships with all departments within the organization to enable interagency collaboration.
Able to accurately and effectively communicate the mission of Housing First, Inc. and its programs, including Coordinated Entry, to community members, stakeholders, partner agencies and clients to promote awareness and understanding.
Participate in events designed to promote awareness of homelessness and / or provision of services to homeless individuals/families.
Case Management
Conduct thorough needs assessment and assist client in developing a Case Plan to address and identify concrete needs and areas of focus, to include: specific discharge plans, linkages and action steps to resolve areas of focus.
Maintain current and accurate documentation of contacts, Case Plan progression, progress notes, outcomes, referrals sent, referral outcomes, and services provided to clients via the HMIS.
Engage in direct personal contact with the client on a monthly basis in order to confirm ongoing eligibility and monitor progression through all phases of Case Plan, within the specified timeframe.
Documents all contacts, progress, and other client level data in the HMIS.
Assist client, as appropriate, in obtaining needed services and mainstream benefits for which he/she is eligible.
Provide client with transportation to/from medical, mainstream benefit and housing related appointments as necessary pending establishment of linkages.
Communicate program policies and expectations to clients and community in a clear and detailed manner and confirms understanding.
Actively assist client in obtaining or maintaining housing independent of the formal social services spectrum via mediation, advocacy, resource linkage, and housing navigation services.
Housing Navigation
Assist clients in leasing the highest quality housing that meets their educational, transportation and employment needs.
Identify and pursue opportunities for systemic change in order to increase housing opportunities for homeless and at-risk individuals and families.
Outreach to community-based organizations, housing resources, and service providers to identify new and existing housing opportunities.
Assist clients in accessing assistance offered by community programs including assistance with security deposits, utilities, moving expenses, housing insurance, etc.
Build relationships with landlords and property owners to advocate for clients and encourage landlord participation.
Work collaboratively with other agencies to develop affordable housing opportunities for clients.
Develop Case Plan and conduct needs assessment to identify and address barriers to locating, obtaining, and / or retaining housing.
Transport individuals or families during the housing search process.
Assist, and coordinate with, other Housing First, Inc. departments as required
Record Keeping
Maintains accurate record of each contact with clients and applicable linkages via the HMIS and hard copy file.
Ensures data completeness and quality rates in the HMIS are consistent with applicable HMIS
Policies & Procedures and electronic records accurately reflect client’s housing status, household composition, and related client level information.
Completes data entry in the HMIS based on the current version of the Coordinated Entry HMIS workflow.
Ensures hard copy files are complete based on program Policies & Procedures and accurately reflect client’s housing status, household composition, and related client level information.
Submits all vehicle fuel receipts, attached to an approved purchase order, to Operations
Coordinator in accordance with Housing First, Inc. Policies & Procedures. JOB REQUIREMENTS:

QUALIFICATIONS AND REQUIREMENTS:
1. Minimum of a Bachelor’s Degree in Behavioral or Social Science; or comparable
2. Good oral and written communication skills
3. Ability to plan and organize self and work to meet goals
4. Must have a valid driver’s license for the state of Alabama and a clean driving record
