Hourly Range: $25 -35 per hour (based on experience)

Job Description:
The Care Coordinator is integral to our medical office, acting as a liaison between patients, healthcare providers, and insurance companies, ensuring a seamless patient experience and efficient healthcare delivery.

Key Responsibilities:

Coordinate and schedule patient appointments, procedures, and follow-ups.
Manage patient records with accuracy and confidentiality.
Facilitate communication between patients, medical staff, and external agencies.
Guide patients in understanding healthcare plans and treatment options.
Handle insurance authorizations, claims, and patient eligibility verification.
Educate patients about preventive care and wellness.
Monitor and track patient outcomes and follow-up requirements.
Resolve patient concerns related to care coordination.

Qualifications:

High School diploma or equivalent
Medical Assistant Certificate or CPR Certification preferred.
Previous experience in a healthcare setting.
Proficiency in healthcare software and electronic medical records.
Knowledge of medical terminology and insurance processes.
