In conjunction with Connections mission to support people with disabilities throughout the community, this position provides mid-level organizational leadership for the agency. Residential Services represents different programming areas that involve supporting people with disabilities in their home environment and in the community. Reporting to the Director of Programs, direct responsibilities include case management of participants in a foster care setting, coordination of medical and mental health services, goal development in conjunction with the program participants, overseeing the coordination of activities of participants, building and maintaining positive relationships with family members, guardians, county workers, and other members of the participant’s team, and staff scheduling. The position also requires the tracking and compilation of all participant billing requirements and maintaining all statute requirements of the program as well as the implementation of programs to support the acquisition of independent living skills. ESSENTIAL FUNCTIONS: 1) Develop appropriate desired outcomes and corresponding objectives and methodologies based on Individual Abuse Prevention Plans, Coordinated Support Service Plans, participant needs, desires, and team recommendations 2) Assist participant in improving independent living skills 3) Maintain open communication with participant’s, family members and/or guardians, county case managers, vocational providers, and other service providers 4) Assist participant in coordinating all medical and mental health appointments i) Attend appointments as needed ii) Provide required data for physicians, including, but not limited to, vital signs and target behaviors. 5) Provide feedback to providers in regards to efficacy of medications i) Monitor side effects and report as needed 6) Monitor all participant programming as stated in program policies 7) Ensure training and support occurs for all participant outcomes as designated in the methodology. Participant programming will be in conjunction with the Minnesota Statute 245D. 8) Maintain participant files in accordance with all program, agency, and licensing requirements i) Keep all necessary forms updated and signed ii) Track all participant goals on a regular basis iii) Collect all relevant data to support necessary documentation iv) Develop and implement new goals semi-annually with the participant 9) Identify the abilities and areas of need for each participant and propose individual program plans to the team to encourage greater independence i) Provide training, support and documentation for all outcomes listed according to established criteria and methods ii) Monitor participant progress through scheduled participant meetings and the preparation of participant reviews iii) Complete all participant assessments as requested 10) Develop and maintain a staff schedule for foster care homes 11) Ensure fiscal management of each house 12) Assure all daily operations of the home are carried out in accordance with policy, practice and licensing requirements i) Assure all participants personal cares are being completed, monitor the health and safety of each participant, report concerns to the team promptly ii) Assure all groceries are ordered, delivered and recorded properly iii) Assure employees are crossed trained at respective supervisor sites iv) Assure all medications are inventoried and accounted for v) Assure all documentation by DSP’s is completed for each shift vi) Assure the home is clean and sanitary meeting all licensing aspects vii) Assure DSPs providing quality care viii) Assure the home is in good condition and report maintenance needs immediately 13) Ensure licensed home remains in compliance, and that all necessary drills, documents, and other requirements are current at all times. 14) Support participant in utilizing community-based resources i) Assist participants in applying for needed community resources ii) Will be responsible to plan, coordinate, and attend participant events, trips and activities as budget permits 15) Supervision of employees as outlined in the Organizational Chart i) Monitor employees performance and complete performance reviews as scheduled ii) Maintain one to one supervision meetings with employees as needed iii) Accurately review staff when to work schedules, mileage logs, and PTO each pay period. iv) Ensure agency standards are being met. v) Hold regular department team meetings. vi) Assist employees in developing department goals and objectives where necessary on annual basis vii) Conduct on-site observations of employees and provide necessary follow-up training 16) Participates in new employee training and/or orientation for all DSPs 17) Provide input in the hiring process for the Adult Foster Care Services Program 18) Participates in all agency sponsored functions/events as directed by Executive Director. 19) Assure proper annual training is achieved in accordance with state statute requirements and employee training plan 20) Prepare, attend and participate in monthly supervision with immediate supervisor 21) Perform other duties as assigned.
