We are looking for drivers ** Drivers will have the opportunity to use a pool car to save you using your own fuel and wear and tear on your own car ** Job description As a Community Care Practitioner (CCP) you will be entrusted and required to provide a high standard of care to all Clients. It will be your responsibility to ensure our ‘Divine’ Clients are empowered to be as independent as possible. As a CCP you will be expected to contribute and follow the Clients’ Personal Care Plans. CCP responsibilities:
To provide the highest quality care to Clients in their own homes in accordance with their care plan, in a person-centred approach
Maintain close contact with Supervisors and Management with regards to the level of service provided to Clients and changes in their circumstances.
Undertake and assist with all personal care including; washing, dressing, bathing and toileting.
Administer, prompt and assist with medication you have to ensure medication is taken correctly and the MAR sheet is completed correctly in conjunction with company procedures. All digital files are updated.
Assist Clients with preparing and cooking meals, essential cleaning and housekeeping
Undertake shopping and prescription errands as required.
Ensure Clients’ notes of service are updated each visit reporting any problems to Management if required.
Qualifications
Proven experience in developing and implementing care plans
Strong mentoring skills to support and guide team members
Ability to assess client needs and tailor care accordingly
Excellent communication and interpersonal skills
Commitment to delivering high-quality care and support to clients
