The Opportunity:
The “Pathway of Unlimited Possibilities,” known as Te Ara Rau Access & Choice (TARAC), operates as part of the Connecting Communities Team. TARAC is dedicated to offering brief psychological interventions and hauora support to all individuals within the Mid Central region, ensuring accessible and comprehensive care.
We invite you to join our team at THINK Hauora as a Health Coach for Te Ara Rau Access & Choice (TARAC).
As a Health Coach, your responsibilities will include:
- Collaborating with tangata whaiora to build active partnerships and motivate them to make positive life changes, reduce risky behaviours, manage stress, and enhance their health and wellbeing.
- Supporting, guiding, and tangata whaiora with health needs and/or disabilities within the community and to connect with service providers to improve health outcomes.
- Identifying the barriers, challenges, and successes experienced by the tangara whaiora
- Collaborating with General Practice Teams (GPTs), Matanga Whai Ora, and Kaiwhakapuaki Waiora to provide coordination, education, and immediate responses to mental health inquiries.
- Encouraging the participation of individuals with health needs and/or disabilities and their families/whānau in various aspects of service delivery and planning.
Key Qualifications and Responsibilities include:
- A strong understanding of Mental Health and/or have a lived experience with mental illness
- A strong commitment to promoting equity, diversity, and social justice in healthcare and community-based settings.
- Ensuring strict confidentiality of all personal and health-related information.
- Demonstrating respect for the cultural and personal values of tangata whaiora.
- Ability to empathise and show compassion towards individuals facing challenges.
- Capacity to identify barriers to healthcare access and develop creative solutions respecting the aspirations of the whanau.
- Willingness to work collaboratively with various stakeholders and alliances.
- Awareness of potential cultural misunderstandings or biases in healthcare settings.
- Ability to advocate for the needs and rights of whānau.
- Clear and effective communication with tangata whaiora, whānau and stakeholders.
- Ability to handle challenging situations and support vulnerable populations effectively.
- The ability to work in a fast-paced role, be a self-starter, have excellent communication skills, and a positive can-do attitude.
- Ability to work independently and as part of a wider team supporting tāngata whaiora wellbeing.
