Mō te tūnga | About the role
We are looking for a Case Manager with a Nursing or Allied Health background, who has the clinical skills required to assist people with current or ongoing mental health problems and to maintain their place in the community. This role will be working within the community and within our integrated model of care.
Our Child & Adolescent Mental Health Service (CAMHS) team is currently looking for skilled and passionate mental health professional. The new role is vital to the mental health of our children and young people across the West Coast as you will be supporting them through their journey along with their whanau. This role will be working within the community alongside our NGO partner Poutini Waiora and within our integrated model of care. There will be opportunities and support for professional development and ongoing role development.
- Undertaking all components of case management, including: comprehensive assessment, risk assessment and management, recovery, and treatment planning
- Providing client-centred care, utilising evidence-based treatment strategies including, where appropriate, talking therapies
- Working Monday to Friday
Ko wai koe | About You
- NZ Registered Health professional as a Nurse, Social Worker, Occupational Therapist, Psychologist or Physiotherapist.
- Excellent communication & relationship building skills, this is a developing role with the opportunity to develop and enhance the service provision and personal practice.
- Passion for child & youth mental health
- Flexibility to work autonomously as well as part of a multi-disciplinary team
- Understanding and application of the principles of the treaty of Waitangi, with a culturally safe and appropriate approach to practice
- Commitment to ongoing professional development
- A strong understanding of Māori health inequity and Māori population need
- Understanding of contemporary approaches to mental health and wellbeing
- Clean & full drivers licence as you will be traveling in DHB cars, this role encompasses the whole of the Westcoast Karamea to Haast.
