Are you a registered health professional looking to work Monday to Friday in an autonomous community-based position?
As a Care Manager no two days are the same, you will undertake assessments in the community using the inter RAI suite of tools and your clinical expertise. Working in partnership with the client, their care giver/family/whanau, you will be developing plans to enable older clients with complex needs to remain safely in the community. Education, and communication with other providers, is pivotal to this role.

To be successful in this role you will have:
- Be a registered health professional (including assessment within your scope of practice). This could be a Registered Nurse or Allied Health Practitioner
- Have a high level of computer literacy
- Have experience in working with older people, and / or chronic care management, in a multi-disciplinary environment
- Be available to work 32 - 40 hrs per week (Monday - Friday)
