Goal Setting and Planning: Collaborate with clients to establish clear, achievable goals and develop personalized action plans tailored to their unique circumstances, preferences, and capabilities.
Crisis Intervention: Offer immediate support and intervention during crisis situations, such as emotional distress, trauma, or urgent mental health concerns, ensuring the safety and well-being of clients.
Resource Referral and Advocacy: Connect clients with appropriate resources, services, or professionals within the community, advocating for their needs and empowering them to access the support and assistance they require.
